Community Services Program Coordinator
NEOGOV is seeking a Community Services Program Coordinator for the City of Glendale (AZ). The role involves developing and implementing programs to assist residents in achieving self-sufficiency, while also connecting them with various resources and services. Responsibilities
Develops and maintains a comprehensive network of education, training, mental health, substance abuse, economic development and other supportive services for youth and adults
Creates and maintains a directory of available service resources
Acts as a liaison between participants and providers
Develops and implements new programs and services for residents and meet objectives as specified in the ROSS Service Coordinator Grant or as requested by BHRA
Identifies needs, defines clear and measurable goals to addresses those needs and connect with resources
Documents participant's progress and re-evaluate services for adequacy
Assists participants in completing all necessary application and enrollment forms
Has regular contact with clients including in person, by phone and in writing
Completes paperwork, monthly reports and case notes accurately and on a timely basis
Coordinates referrals to and/or provides on-site supportive services for ROSS participants including but not limited to mental health, GED programs, employment workshops, financial education, substance abuse with appropriate community agencies
Pro-actively develops and promotes self-reliance activities for families including but not limited to Financial Counseling, Homeownership and GED programs
Arranges for education sessions on such topics as tenant rights and responsibilities; elderly abuse; aging-in-place and maintaining independent living
Seeks out new program funding sources, write for public and private grants and work toward program sustainability
Coordinates with the FSS program as needed to ensure fluid transfer of participants between programs to support resident progress toward self-sufficiency
Evaluates needs and gaps in services and work to identify solutions
Assists with monthly Resident's Council meeting and resident activities
Operates city vehicles
Performs other related duties as assigned
Qualification
Required
Associate degree in related field such as Social Services, Psychology, Human Services, Counseling or related field and two years' experience in case management or related work
Knowledge of methods and techniques of effective case management and assessment skills
Understanding of home buying process, credit scores, and financial literacy issues
Knowledge of community social service resources
Knowledge of various social service program guidelines and eligibility requirements
Knowledge of human behavior and emotional problems
Knowledge of available community resources and referrals and outside service agencies
Knowledge of financial problems and concerns of low-income families and individuals, especially the elderly and disabled
Ability to write grants, grant reports, generate promotional material and public relations releases
Ability to effectively deal with clients, often in difficult circumstances, and obtain their cooperation and the necessary information quickly
Ability to handle confidential and sensitive issues in a politically correct manner
Ability to gather and review information and develop sound conclusions and eligibility determinations
Ability to maintain complete and accurate records and case files
Ability to learn and interpret new regulations, guidelines, and procedures
Ability to maintain effective working relationships with participants, property owners, landlord's co-workers, and the general public
Ability to operate city vehicles
Valid AZ Driver's License
This position is classified as safety-sensitive which requires candidates to successfully undergo drug and alcohol testing prior to employment
Candidates will be subject to further drug and alcohol testing throughout their period of employment in accordance with the City of Glendale Substance Abuse policy
A post-offer physical assessment is required
Preferred
Bachelor's degree is highly preferred
Four years of experience in case management or related work in the field of Social Services, Psychology, Human Services, or Counseling
