Program Support Assistant - Case Management
Swedish is a leading healthcare organization dedicated to patient care. The Program Support Assistant - Case Management will support the Case Management Team by assisting in patient placement and collaborating with various departments to ensure continuity of care.

Responsibilities
Participate as an active member of the Case Management Team (CMT)
Under the guidance of the nurse case manager and social worker, assist in facilitating appropriate patient placement
Utilize hospital and community resources to support continuity of patient care
Serve as a Utilization Management liaison
Collaborate effectively with Medical Staff, CMT members, the Business Office, Medical Records, and other hospital departments
Perform other duties as assigned
Qualification
Required
1 year of job-related experience in a healthcare setting
Preferred
Associate's Degree
Bachelor's Degree
Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology upon hire
Prior experience in discharge planning, billing, utilization review, financial counseling or registration
Proficiency in medical terminology
