NEOGOV is seeking an Aging Care Manager 2 for the Lebanon County Area Agency on Aging. The role involves coordinating the PA MEDI program, promoting Medicare education, and providing support to Medicare beneficiaries and their families.

Responsibilities

Creating community awareness and promoting the PA Medi program through speaking engagements, enrollment events, informational meetings, and various marketing and public relations activities
Developing and maintaining cooperative working relationships with community partners and other agencies and organizations, including recruiting, training, and coordinating volunteers
Providing counseling and education to all beneficiaries and their family members pertaining to Medicare, Medicaid, supplemental health insurance, and prescription drug plan options
Act as an intermediary between Medicare beneficiaries and their health insurance and prescription drug plans to resolve issues and assist with appeals
Provide current material for Lebanon County Area Agency on Aging website

Qualification

Required

Six months as an Aging Care Manager 1 or a County Caseworker 1; or
Successful completion of the County Social Casework Intern program; or
Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
An equivalent combination of experience and training
You must meet the PA residency requirement
You must be able to perform essential job functions
A conditional offer of employment will require submission of criminal history reports
