Responsibilities
Conduct outbound calls to enroll members into Case Management programs
Maintain an average of 60-70 calls per day
Serve as a point of contact for member and provider inquiries
Support members with care plan next steps and available community/health plan resources
Provide telephonic support regarding scheduling, services, and program education
Assist with onboarding activities including sending welcome letters and program materials
Document and maintain accurate non-clinical member records in accordance with state and regulatory requirements
Identify and refer members to appropriate resources, including those addressing Social Determinants of Health (SDOH)
Perform other administrative duties as assigned
Qualification
Required
Experience with high volume of calls
Experience in healthcare or managed care environment
Proficient in MS Office Applications
1-2 years of experience in a managed care, medical office, healthcare facility, or similar setting
Experience handling high-volume calls
Proficiency in Microsoft Office
Strong customer service and communication skills
High School Diploma or GED
