Responsibilities
Conduct comprehensive clinical assessments to determine home care eligibility and safety
Develop, implement, and manage patient-centered care plans
Collaborate with physicians, patients, families, and interdisciplinary care teams
Monitor patient progress and update care plans as needed
Provide supervision and oversight of Home Health Aide (HHA) services
Participate in interdisciplinary case conferences
Ensure accurate and timely clinical documentation using Homecare Homebase (HCHB)
Maintain compliance with CHHA, state, and federal regulations
Qualification
Required
Active New York State RN License and current registration
Associate Degree in Nursing required
Minimum 2 years of experience in home care patient assessments and care planning
Minimum 1 year of supervisory experience in a hospital or home care setting
Proficiency in MS Office and electronic documentation systems
Valid driver's license
Preferred
Bachelor's Degree
PRI assessment experience
Experience with Homecare Homebase (HCHB)
Bilingual skills
