New Grad, Entry LevelSt. John's Residence for Boys is a dedicated Residential Treatment Center serving young men within the ACS system. The Case Planner will provide case management services, assess client needs, and develop comprehensive service plans to ensure appropriate resources are delivered to clients.

Responsibilities
Provide case management services, including assessing client needs, developing comprehensive service plans, and ensuring the delivery of appropriate resources
Maintain regular communication with clients and their families
Coordinate with other service providers
Document all interactions accurately and promptly
Advocate for clients' needs and support them in achieving permanency
Qualification
Required
Strong case management, assessment, and service planning skills
Effective communication, active listening, and conflict resolution abilities
Knowledge of child welfare systems, family dynamics, and community resources
Proficiency in documentation, organizational skills, and time management
Ability to work independently, as well as collaboratively with a team
Bachelor's degree in Social Work, Psychology, or a related field is required
Preferred
Master's degree is preferred
Experience working with children and families in diverse communities is a plus
Bilingual proficiency is a strong asset
