Jail Diversion Coordinator

The Jail Diversion Coordinator is responsible for conducting assessments and providing intervention and referrals for justice-involved individuals living with serious mental illness or substance use disorders, working closely with detention center staff and community stakeholders.

Responsibilities
Process requests for services by conducting intakes, obtaining /completing necessary assessments, social histories, and collecting documentation for the determination of program eligibility
Monitor the database of incoming referrals from both High Point and Greensboro Jails and coordinate engagement with them
Attend court hearings as assigned. Collaborate with the District Attorney’s Office/Public Defender
Assist with mental health and substance use grants
Provide accurate and thorough reports to hospitals after involuntary commitment
Provide appropriate service referrals to clients and/or family members either through the direct assessment process or when requests are made by phone
Act as liaison and consult with GCSTOP, Guilford County Reentry Programs and additional community agencies to maintain coordination of the intervention process. Network with appropriate organizations within Guilford County and maintain positive relationships with law enforcement agencies in Guilford County
Complete required documentation within 24 hours, to include case documentation, assessments, notes, etc
Collect and provide necessary data to quantify and evaluate outcome measurements
Provide input in developing protocols for program sustainability and enhancement
Assist with program development and expansion in Guilford County
Provide the director with information regarding new referrals for coordination and updates on open cases
May provide direction and leadership for interns, volunteers, or contract employees. case managers
Develop and manage a stakeholder network of external partner agencies
Serve as an advocate for the individual and facilitate the Interdisciplinary Team in the development of the Individual Comprehensive Plan
Coordinate, implement, and monitor the Individual Comprehensive Plan
Provide mediation between the individual and the providers, guardians, family members, or the agency relative to the needs and desires identified by the individual
Assist in accessing community resources identified for the individual, family members, and/or providers
Participate in professional development opportunities
Qualification
Required
Master's degree in social work, counseling or human services related field and one year of experience in mental health or substance use; OR, Bachelor's degree in social work, human services related field and three years of experience in mental health or substance use
Valid Driver's License
Technical Knowledge: Computers and electronic data processing, Word processing software (i.e. Microsoft Word), Email (i.e. Outlook), Internet, Electronic record keeping system, Jail Management Software
Math Knowledge: Basic math such as adding, subtracting, multiplying, and dividing
Personal Skills: Flexibility in adjusting to changing circumstances, information, personnel, and customer needs, Demonstrate a willingness to take initiative and work independently, Understanding of and ability to practice safe working habits, Brings others together to reconcile differences, High level of integrity
Technical Skills: Basic typing skills, Excellent communication skills, Good writing skills, Ability to learn how to use the County's email, internet and other systems
Organizational Skills: Effective time management and organizational skills
Communication Ability: Ability to be culturally competent/humble with diverse populations, Ability to communicate effectively to various groups of individuals in person, in written form and via telephone, Positively represent the Guilford County Sheriff's Office to customers, the public, other entities and external sources, Develop/maintain constructive and cooperative working relationships with others, Use courtesy, discretion, and sound judgment, Effectively meet and communicate with customers and the public
Technical Ability: Use computer systems including hardware and software to set up functions, enter data, process information, Observe, receive, and otherwise obtain information from relevant sources, Understand and interpret legal and government documents such as Guilford County Sheriff's Office Policies and Procedures, Comprehend and apply technical information
