Homecare Referral Coordinator, case management, customer service, homecare referral systems
Korean Women's Association (KWA) is a non-profit multicultural social service agency dedicated to providing multi-lingual human services. The Homecare Referral Coordinator will manage the intake and follow-up of homecare referrals, ensuring timely responses and maintaining accurate records while contributing to improved service access and client advocacy.
Responsibilities
Ensure timely, professional, and coordinated responses to both referral sources and prospective clients
Maintain accurate and comprehensive records of all referral activity
Collaborate with regional teams to enhance patient/client advocacy and streamline case management processes
Qualification
Required
Experience with homecare referral systems and homecare contracting in Washington State
Knowledge of Long-Term Services and Supports (LTSS) and community-based care programs
Associate degree in healthcare administration or social services or related field
Two (2) years of experience in healthcare or homecare or case management intake coordination
Preferred
Bachelor's degree in healthcare administration or social services or related field
Two (2) years of experience in healthcare or homecare or case management intake coordination preferred
