Specialty Program Coordinator- Infectious Disease

Responsibilities

Works as the coordinator between physician offices, specialty clinics, hospital departments and patients to promptly meet the needs of patients scheduled for clinic appointments and procedures as well as referrals across the organization. Serve as an essential link between patients and all other care providers
Maintains the assigned minimum number of patient cases longitudinally. Tracks and maintains large amounts of data such as patient profiles, appointments and interactions
Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team, and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines
Routinely collaborates with others to assess gaps related to patient outcomes with integrated efforts towards continuous quality improvement and refinement; compiles statistics/data as requested and shares with management as directed
Contacts patient prior to appointments to confirm appointment and identify barriers or concerns that may interfere with their preparation and arrival to their scheduled appointment; obtains, records and retrieves necessary patient information using the Medical Information system; prepares care team for patient arrival; enters test orders and insurance authorizations where applicable; where applicable mails any specimen/sample kits to patient pre- or post-visit
Assists patients and his/her family by identifying resources as part of their care plan in areas such as but not limited to medical treatment, insurance or affordable alternative programs, financial and social issues. Provides referrals for services to community agencies as appropriate
Follows up with patients and families/significant others (where appropriate) throughout care plan; where needed escorts patient and/or their families/significant others to pre- and post-procedure areas
Identifies opportunities for improvement in patient care and organizational processes
Anticipates the needs of all customers (internal and external) and assumes responsibility for solving customer problems when appropriate
Contributes to performance improvement of the hospital, the clinic, and patient care team
Ability to perform functions using job-related software and systems
Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients
Cultivate positive working relationships with clients
Complete all required trainings, submission of invoices, and other required duties in a timely manner and in accordance with standard operating procedures
Assist eligible clients in completing relevant applications, securing necessary supporting documentation, and filing the applications with the appropriate agencies
Provide referrals to and work collaboratively with onsite Case Managers
Work with Patients to schedule and complete appointments, provide holistic support to increase patient self-management, and assist in achieving medical stability
Work collaboratively as part of a Ryan White Service team
Provide Patients with the information, resources, and support they need to remain in care, adhere to their medication, and maintain good health and well-being
Help Patients to access services from multiple providers, acting as a bridge between patients and providers
Coordinate scheduling of non-emergency medical transportation via comprehensive Lyft program
Assist with linkage and navigation of HIV Pre-exposure Prophylaxis (PrEP) Care

Required

High School diploma or equivalent
one year of customer service or medical experience
equivalent combination of education and experience
Familiar with guidelines regarding patient confidentiality issues
Must have working knowledge of MS Office platforms such as (Excel, OneNote, Word, PowerPoint)
Ability to convey information both in writing and verbally
Must have high level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service
Must have sensitivity to, interest in and competence in cultural differences, HIV/AIDS, sexual practices, substance dependency
demonstrated competence in working with the gay/lesbian/bisexual, transgender community
