Clinical Program Coordinator - Stroke

The Clinical Program Coordinator-Stroke is responsible for coordinating clinical, regulatory, and performance improvement activities related to stroke patients, ensuring compliance with accreditation guidelines and promoting continuity of care.

Responsibilities
Responsible for programmatic coordination of clinical, regulatory and performance improvement activities related to the stroke patient population
Accountable for implementation of the Comprehensive Stroke Center's program goals
Developing and maintaining systems that ensure clinical, operational and service quality goals have a process in place to achieve the relevant standards
Instrumental in the continued development and implementation of the organization's stroke accreditation program
Compliance with The Joint Commission (TJC) Accreditation Guidelines, CMS Conditions of Participation and other state and federally mandated regulatory agency guidelines as appropriate
Functions as the stroke system coordinator for activities relating to patient and family education, promotion of continuity of care, community education, staff education, and committee work related to the stroke program
Data collection, analysis and reporting for the purpose of advancing the stroke program's clinical and academic goals
Qualification
Required
State of Vermont Registered Nurse (RN) license required
Five years of relevant nursing experience
Preferred
Master's in related field considered strongly
Stroke certification preferred
