Early Intervention Service Coordinator

The North Carolina Department of Health and Human Services is one of the largest agencies in the state, responsible for health and human services. Responsibilities

Conduct initial contacts with families to explain the NC ITP, obtain consents, and gather intake and financial information
Coordinate and facilitate eligibility evaluations and assessments, ensuring compliance with procedural safeguards and multidisciplinary requirements
Develop functional IFSPs in collaboration with families and team members, identifying outcomes, services, and appropriate providers
Authorize and monitor early intervention services as outlined in the IFSP, ensuring quality and adherence to best practices
Conduct regular IFSP reviews and updates based on child progress, family input, and service provider feedback
Assist families in accessing and navigating community resources, including medical, developmental, and social supports
Provide consultation and technical assistance to community partners and service providers involved in early intervention
Support families through the transition process as children approach age three or exit the program, including referrals and planning meetings
Maintain accurate and timely documentation in compliance with NC ITP data and reporting requirements
Participate in quality assurance activities and collaborate with CDSA staff to ensure effective service delivery

Required

Bachelor's degree in Special Education, Elementary Education or Early Childhood Education, Psychology from an appropriately accredited institution and two years of professional experience working with the population served
Bachelor's degree in human services field from an appropriately accredited institution and three years of professional experience working with the population served
an equivalent combination of education and experience
Must be able to drive and travel by car across the CDSA's defined catchment area

Preferred

Proven experience coordinating multidisciplinary evaluations and assessments for infants and toddlers, including scheduling, documentation review, and integration of results across all required developmental domains
Documented experience developing and implementing Individualized Family Service Plans (IFSPs) in collaboration with families and service teams, with clearly defined, measurable outcomes based on identified needs
Demonstrated experience monitoring early intervention service delivery through provider documentation, direct observation, and family engagement to ensure quality, compliance, and progress toward IFSP goals
Hands-on experience using data systems and documentation tools to maintain accurate, timely records and support required reporting for federally regulated early intervention programs
Experience facilitating transitions for children exiting early intervention services, including coordinating with receiving programs, conducting planning meetings, and ensuring procedural safeguards are followed
