Responsibilities
Accurately enter and update patient, clinical, and billing data into Electronic Health Record (EHR) and administrative systems while meeting daily productivity targets
Perform routine data validation and reconciliation to identify and correct discrepancies between source documents and digital records
Maintain strict confidentiality by following HIPAA policies and secure data-handling procedures for all patient information
Collaborate with nurses, social workers, and billing teams to clarify documentation, resolve missing information, and support chart completion
Generate and deliver regular accuracy and completion reports to supervisors; escalate critical data issues promptly
Support process improvement initiatives by documenting common data errors and recommending solutions to streamline intake and documentation workflows
