Clinical Pharmacist -Outpatient
USAJOBS is seeking a Clinical Pharmacist for the Veterans Health Administration. The clinical pharmacist will handle routine medication-related activities, serve as a medication expert, and manage medication therapy across the continuum of care while planning and implementing drug monitoring programs.

Responsibilities
Responsible for handling routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN) and national polices and regulations
The clinical pharmacist is the medication expert in all assigned areas, functions as a consultant to other health care professionals and manages medication therapy across the continuum of care
Plans, implements and operate drug monitoring and utilization programs as assigned
Responsible for developing, providing, and supporting educational programs of the Service and Medical Center
Exhibits initiative and resourcefulness in managing assigned workload
Provides high quality, patient centric care using evidence-based medicine to ensure safe, effective and efficient medication therapy
Interacts professionally with all customers
Incorporates the VHA mission, vision and goals into own priorities and practice
Maintains current, comprehensive knowledge of evidence-based practices and actively pursues opportunities for continuous professional development
Qualification
Required
U.S. Citizenship
Proficient in spoken and written English
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (PharmD.) degree
Graduates of foreign pharmacy degree programs must provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Full current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia
1 year of experience equivalent to the next lower grade level, or completion of an ACPE-accredited PharmD. Program
Knowledge of professional pharmacy practice
Ability to communicate orally and in writing to both patients and health care staff
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
