Responsibilities

The Intensive Case Coordinator is responsible for organizing, mediating and documenting the Child and Family Team meetings and assisting families with the CFT process when necessary. Mediate responsible and responsive discussion regarding flex fund requests with youth and family as necessary. Track progress on action steps throughout the treatment process
Demonstrate the ability to work as part of a multidisciplinary team. This includes working with the family members and their identified natural supports, as well as assigned agency team and staff from DMH, DCFS, Department of Probation, psychiatrists and other interagency and intra-agency staff to provide consistent services and
Completes required SCHARP check request forms for processing Flex Funds, forwards to supervisor for review and approval
Develops client Safety and Crisis plans as needed in collaboration with the Child and Family Team. Works with the Team to coordinate and monitor freatment progress and Safety Plans to ensure services are coordinated and pertinent to the child and family's needs, desires, goals and objectives
Maintain contact with family members and significant other support persons of clients as appropriate. Involve significant support persons at the request of the youth and family receiving mental health services
Maintain a UOS log daily for all services rendered, documenting all services to be billed to LACDMH. Service codes and time billed must be consistent on the UOS log and progress note. Documentation is to be completed and submitted within 48 hours of direct service contact. Under no circumstances should billing sheet be submitted for data entw without the progress note being completed
Involve child/ youth and their families in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible
Ensure child / youth and their families are involved in establishing goals in the areas of living arrangements, social relationships, constructive use of leisure time, productive activity, and dental and medical health care
Attends and participates in all pertinent SCHARP, DCFS, DMH and/or Department of Probation meetings for each client as necessary
Be available to respond by phone and in person as necessary to crisis calls as needed based on an on-call rotational schedule

Required

High School Diploma or equivalent
Bachelors Degree in Social Work, Psychology or other human services field
Must have at least 2 years experience (3 years experience without a Bachelors degree) working with high risk children/youth in either Child welfare, probation or mental health systems
Bilingual English/Spanish
