Population Health Program Coordinator

Responsibilities
Responsible for reporting, data collection and information distribution on projects integral to sustaining quality improvement (QI) initiatives, and payer programs related to population health efforts
Manages disease-based dashboards and registries
Identifies care gaps in collaboration with appropriate clinical leadership
Coordinate strategic efforts in addressing the population health care gaps
Works with data analyst to run reports on chronic disease management metrics and utilization
Works with data analyst to maintain and develop monthly reports of key elements needed for the projects and feedback to the practice on progress
Coordinate process improvement projects and provide implementation support to care teams
Provide project management support for the department
Reviews and completes reports for city, state and federal agencies, including payers, as directed
Completes and submits monthly reports
Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
Other duties as assigned to support the unit, department, entity, and health system organization
Qualification
Required
Bachelor's Degree (Required)
2+ years of experience in Healthcare (Required)
Preferred
Master's Degree in Public Health (Preferred)
