Clinical Pharmacy Specialist (Acute Care)
The Memphis VA Medical Center is part of the Veterans Health Administration, and they are seeking a Clinical Pharmacy Specialist (Acute Care). Responsibilities
Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and formulary management. Documents interventions as necessary
Participates in multidisciplinary rounds with the healthcare team(s)
As part of the interdisciplinary team, appropriately render care to patients, including but not limited to prescribing and/or altering medications, ordering and analyzing necessary lab tests, and assisting in the management of medical emergencies, under the CPS's Scope of Practice
All inpatient/outpatient orders are checked to include patient information, correct medication, dose, route, and frequency of administration. Orders are checked for patient allergies, drug interactions, incompatibilities, duplication and priority
Dispenses controlled substances with accuracy according to current policy and procedures for inpatients and outpatients
Serves as an authoritative information source on drugs and their uses, with special emphasis on agents which require therapeutic monitoring
Assist in preparing patients for discharge by evaluating the patients' medications, ordering necessary medications and providing education to the patient and/or caregiver
Provides oral and written consultation with other health care professionals regarding drug therapy selection or management (including drug induced problems) in an accurate, timely and professional manner. Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen
Supervises and trains pharmacy residents and students in acute care. Evaluates residents and students based on performance, presentations and examinations. Provides in-service presentations on acute care topics to students, residents, and professional healthcare staff. Maintains an organized and structured acute care rotation for pharmacy students and residents
Educates patients on the proper use and care of their medications through the day-to-day personal interactions with the patients. Documents patient education and counseling in medical record. Patient education may occur in a group clinic setting
Correctly interprets and adheres to all hospital and Pharmacy Service policies and procedures
Consistently responds to patients, family members, visitors, and co-worker requests for questions or assistance in a prompt, timely and cooperative manner
Qualification
Required
U.S. Citizenship
Proficient in written and spoken English
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Knowledge of Professional Pharmacy Practices

Part-time Experience
1 year of experience equivalent to the next lower grade level (GS-12)
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Preferred
PGY-1 Pharmacy residency experience
PGY-2, board certification, or experience preferred
