Responsibilities

Manage and maintain a maximum caseload of 15 families who are DCFS involved
Display effective inter-personal skills, a dedication to helping youth and families reach their goals, and a strong attention to detail in the coordination of services and care
Collaborate with families to identify individualized needs and develop a plan of care to meet those needs
Complete a strength-based assessment of all life domains identifying both strengths and needs
Facilitate and lead the Family Team in all meetings, ensuring the youth and family voice is heard and respected by all team members
Confidently lead and engage in effective family team meetings, while being observed by program supervisor or program funders
In collaboration with the Family Team, develops a comprehensive plan of care, assigning responsibilities to each team member to address needs including both non-traditional services and evidence-based treatments, as appropriate
Guide the team to adjust the plan to address any anticipated crisis situation
Provide consultation and support to the Family Team in the implementation of the plan of care and monitoring the progress toward goal achievement
Advocate for the family, especially around meeting their needs in the least restrictive setting possible
Completes the Child and Adolescent Needs and Strengths (CANS) Assessment tool at designated intervals
Maintain all administrative tasks in a timely manner through Google Drive, Apricot online database, ETO case notes (the data management system) and on-site case files
Adhere to the fidelity of Wraparound facilitation, as taught during on-the-job training. (NCFIE and MiiWrap)
Home visits and meetings with families within their community (Cook, Lake, and/or Will County) is required
Engage in ongoing professional development and maintain up to date knowledge of DCFS policies

Required

Bachelor's Degree required: in the field of Counseling, Psychology, Criminal Justice, Social Work, Education or related program
Experience coordinating care and/or working with youth and families on a direct service level
Sensitivity to cultural diversity is required
Ability to communicate and cooperate with diverse families, various professionals and community groups
Assertive personality willing to follow up with non-engaged youth, family, and supports ensuring tasks are completed promptly
Attention to detail and thoroughness
Proficient computer skills in Microsoft Windows and internet proficiency, including the use of Google Drive applications
Effective verbal and written communication skills
Must have a vehicle with a valid driver's license and automobile insurance required
Must be able to remain stationary 50% of the time
Must be able to frequently move inside the office to access file cabinets, office machinery, etc
Must be able to transport oneself to and from home visits
Occasionally ascends/descends stairs
Constantly moves to assist during program activities
At times it may be required to lift up to 25 lbs. for various needs
Frequently moves in outdoor weather conditions

Preferred

Experience working with diverse populations and in diverse communities
Basic knowledge of the Illinois child welfare; knowledge of local resources and service providers
Knowledge of the DCFS SACWIS data system and the Child and Adolescent Needs and Strengths (CANS) Assessment
Strong collaborative, relationship building, and communication skills to effectively resolve problems, achieve goals, and meet deliverables
Bi-lingual Spanish speakers
