Responsibilities

Act as a liaison between community organizations and pharmacy teams to further develop, support, and sustain collaborative relationships
Refine systems and processes that enable scalable outreach, education, and community connections
Coordinate multi-site initiatives and contribute to program development, implementation, and evaluation for grant-funded activities
Distill and disseminate information for communication with PSW members, payers, community partners, and families served
Ensure program operations adhere to federal, state, and health-system regulatory requirements, organizational requirements, and federal and state reporting requirements

Required

Excellent project management skills
Demonstrated experience supporting or managing outreach programs in healthcare
Proven ability to creatively design and refine systems and processes that support scalable, compliant program operations
Strong writing and communication skills for varied audiences, including federal and state grant reporting
Skilled in Microsoft Office Suite (Word, Excel, Teams, PowerPoint)

Community Health Worker-trained individuals encouraged to apply
