Responsibilities

Conduct outreach to members via phone to support care plan next steps, community resources, and health plan inquiries
Answer inbound calls, schedule services, and provide accurate information to members and providers
Document member interactions and care activities in accordance with state and regulatory requirements
Meet deadlines for assessments while ensuring accuracy and compliance in documentation
Maintain effective communication with providers, members, and internal teams to improve health outcomes and enhance member experience
Support the Care Management team in reducing costs and improving overall member satisfaction

Qualification

Healthcare experienceCustomer service experienceAdministrative support experienceEffective communicationDocumentation skills

Required

Education: High School Diploma or GED (required)
Experience: 2–4 years of relevant experience in healthcare, customer service, or administrative support
