Collaborative Care Coordinator

Texana Center is a healthcare organization seeking a Collaborative Care Coordinator to provide direct, coordinated services to individuals with co-occurring intellectual and developmental disabilities and mental illness. The role involves comprehensive case management, skills training, and crisis intervention to promote recovery and stability for clients.

Responsibilities

Assessing initial levels of functioning using standardized assessment tools and monitoring progress to evaluate overall improvement
Conducting comprehensive biopsychosocial assessments to identify areas of need, prioritize treatment goals, and determine appropriate supports
Developing and maintaining a Person-Centered Recovery Plan (PCRP) that guides treatment goals, skills development, and addresses barriers to success
Coordinating referrals, tracking follow-up, and consulting with mental health providers, specialized therapy clinicians, and medical service providers
Providing mental health wellness support and skills training aligned with foundational needs identified in the PCRP
Assisting with individuals with medication management and facilitating access to mental health and specialty services
Ensuring adherence to, and promotion of, Texana’s Compliance Policies and procedures
Travel throughout a 6-county service region
Maintaining a standard work schedule of Monday – Friday, 8:00am – 5:00pm, with flexibility to accommodate client needs

Required

Meet the requirements as a QIDP and a QMHP: 1 year experience working with persons dually diagnosed with IDD and a mental illness
2 years of experience working with individuals with IDD who have mental health and behavior support needs or linking people with IDD to mental health supports
1 year of experience working directly with individuals with IDD or other developmental disabilities
Bachelor's degree in health and human services field
Texas Driver License (in good standing)
Must be able to perform SAMA and CPR. Training is provided to employees once hired
Must be able to perform moderate to heavy lifting (15#- 45#), kneeling, pushing, stooping, climbing stairs, walking, standing, and sitting
Must have ability to see, write, operate office equipment, operate a motor vehicle and depth perception
Qualified individuals must be able to perform these functions with or without reasonable accommodation

Preferred

Experience in crisis prevention and management
Spanish speaking
