Lead Holistic Support Manager- Quality Supports

Responsibilities
Ensure that individual rights and dignity are respected by actively supporting the agency's mission, vision, and values
Provide Person-Centered Planning and Holistic Support Management in Meaningful Day opportunities for people supported and be available as a resource for the HSM Team to troubleshoot daily PCP-related matters and funding choices and options
Perform daily monitoring of LTSS for pending PCP approvals and connect with HSMs to troubleshoot any clarification requests or interventions needed to approve PCPs
Work with the HSM Team to identify and communicate strengths and barriers to people supported and join the leadership and Quality Enhancement (QE) effort to ensure that all PCPs connect to their identified needs such as the goals and outcomes, Caretracker, Health Risk Screening Tool (HRST), iCare Manager (iCM), etc
Assist with auditing the accurate completion of PCPs and all related components according to the defined timeline and assisting the HSM Team to adhere to all PCP requirements for plan compliance
Develop a tracking system and ensure that all needed billing and compliance measures related to the PCP, such as Caretracker functions and quarterly PCP reviews, are in place and available for audit purposes
Collaborate with agency support teams such as the HSM – Community Living Team, Coordinators of Family and Independent Living, Meaningful Day Team and the Director of Clinical Supports to ensure that all services are accurately reflected on the Detailed Service Authorization Tool (DSAT) so each person’s PCP is planned holistically and includes all needed services
Attend all meetings and training courses, as required
Assist with The Arc Baltimore’s fundraising events
Actively participate in The Arc Baltimore’s committees and work groups
Perform other duties as assigned
Qualification
Required
Associate degree in Social Work, Psychology, Special Education, or a related field
Five years or more of case management or supervision experience in the disability field is required for this position
Knowledge of Microsoft Office is required
Knowledge of LTSS, PCIS, iCM, and HRST systems required
Valid Maryland driver's license required and approved clean driving record necessary
Must have access to a personal car to conduct agency business, including the transportation of people supported by The Arc Baltimore
Some lifting up to 50 lbs
Must be available during scheduled work hours by use of a personal cell phone
Must work effectively within a team environment
Preferred
Bachelor's degree preferred
In lieu of a degree, ten or more years of relevant experience in the field may be considered as a substitute
