Admissions Coordinator

They are seeking an Admissions Coordinator responsible for conducting comprehensive admission assessments, initiating individualized care plans, and ensuring a smooth admission process in collaboration with the interdisciplinary team.

Responsibilities

Conduct comprehensive admission assessments for new residents and hospital returns, and initiate the development of individualized care plans
Support the Nursing Department’s development and operations under the direction of the Director of Nursing (DON)
Evaluate PRI assessments and coordinate the full scope of the admission process
Uphold confidentiality of all resident information in compliance with HIPAA and company guidelines
Assess inquiries and respond to referrers as a priority
Work with the interdisciplinary team to coordinate client flow
Ensure an immediate response to phone, fax and walk- inquiries and referrals
Coordinates tour, application, and sales process with customer, families and referrers
Meet with client and families on day of admission. Facilitate an initial introduction to the nursing staff on the unit
Communicate admission criteria to referral sources, clients and families
Work with administrator and appropriate staff to ensure consistent, effective admission, discharge, and transfer meetings
Maintain statistics indicating potential client flow, pending and lost inquiries, denials, hospital discharges, and marketing information
Work to initiate, nurture, and maintain contact with physicians, social workers, and discharge planners
Coordinate and participate in presentations about the facility to hospitals and physicians
