HIM Data Integrity Specialist
SouthEast Alaska Regional Health Consortium (SEARHC) is a non-profit health consortium serving the health interests of Southeast Alaska residents. The HIM Data Integrity Specialist is responsible for ensuring the timely, complete, and accurate inclusion of all documentation in the electronic health record, performing audits, and managing documentation compliance.

Responsibilities
Proficient in all job competencies included in the HIM Technician job descriptions and provides coverage of these duties as needed and appropriate
Analysis and review of patient records
Performs analysis of inpatient, observation, same day surgery (including infusion clinic), swing bed, and emergency department encounters to assess completeness of documentation
Ensures the physician/provider properly signs, dates, and completes all provider documentation in the record
Appropriately assigns deficiencies and notifies providers of any missing/incomplete documentation following Joint Commission standards and consortium policy
Reviews deficiencies for completion daily and completes records out of the analysis queue as missing/incomplete documentation is present
Performs audits of documentation as requested by supervisor
Assists with chart requests for legal audits working closely with the Compliance Officer and legal department to ensure records produced meet the needs of the request
Reviews the HIMCORE email and processes any received requests
Manages transition of care documents from partner facilities
Completes the Arista MD and Bartlett processes
Works closely with the Privacy Officer to ensure all activities align with HIPAA and other relevant regulations such as record reviews, record corrections, etc
Performs routine audits to identify electronic health record errors and duplicate charts; helps with chart merges
Establishes and maintains Key Performance Improvement measures, is responsible for data abstracting, running quality and statistical reports, supporting the revenue cycle team with documentation compliance
Under direct supervision of the HIM Operations Supervisor and/or HIM Manager, makes corrections to the Electronic Health Record (EHR) documentation as requested through the helpdesk by clinicians, providers, physicians, and other EHR users
Works closely with Compliance Officer following Federal Privacy Act and HIPAA, ensuring compliance
Under the guidance of supervisor, audits encounters of fellow team members and shares corrections that need to be made
Provides performance improvement feedback to fellow team members ensuring supervisor included is in communications; trains team members as needed and appropriate
Performs review of obituaries and other submitted notifications of death and updates the EHR system as appropriate, scanning supporting documentation to the patient’s case management encounter
May support other locations with registration and scheduling activity

Qualification
Required
High school diploma or equivalent
Two years' HIM experience, including managing paper and electronic medical records and computer databases
Two+ years' computer/office experience in a health care setting working with medical terminology and medical records
Years of experience may be replaced by associate's degree or higher in HIM or related field

Knowledge of workflow and processes related to both electronic and paper medical records within a Health Information Department
Knowledge of multiple medical specialties
Knowledge of IPAA Policies and regulations and knowledge to complete work in compliance of these standards
Knowledge of medical record content and sequence
Effective verbal and written communication, including proper English spelling, grammar, and punctuation, and documentation formatting
Strong organizational, analytical, and interpersonal skills
Accurate and timely detailed work in analyzing and maintaining patient care data
Ability to prioritize work assignments and accomplish quality work within set time limits
Ability to access and work with multiple electronic systems
Ability to effectively handle regular interruptions of task and the pressure of multiple demands
Ability to work independently with minimal supervision
Ability to understand and follow oral and written directions
Proficient in Microsoft Office Products including Word, Excel and PowerPoint
Proficient in other SEARHC provided computer applications
Proficient in data entry, document imaging and abstracting
