The Marion VA Health Care System is seeking a Full-Time Registered Nurse (RN) to serve in the Geriatrics and Extended Care Community Care section of Care Management & Social Work Service in Marion, IL or Evansville, IN. The RN will be responsible for executing an efficient approach to receiving, triaging, and directing timely coordinated care while collaborating with members of the interdisciplinary team to influence care outcomes.

Responsibilities

The RN is responsible for executing an efficient approach to receiving, triaging and directing timely coordinated care
The incumbent must demonstrate proficient practice and decision making, deliberate planning, and critical thinking skills
The incumbent possesses intimate knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines
The incumbent will influence care outcomes by collaborating with members of the interdisciplinary team both within the Veterans Affairs (VA) and in the community
The incumbent will comply with VA-mandated processes and use of health information solutions that serve to assess, document, exchange, and track care coordination information
The RN will utilize delegation of authority, if applicable, to function at their highest level of role performance and align high-quality care with key performance indicators to ensure practice standards meet patient care needs

Registered Nurse LicenseBLS CertificationProfessional Nursing EducationCommunity Care ExperienceEnglish ProficiencyCritical ThinkingCollaboration Skills

Required

Graduate of a school of professional nursing approved by the appropriate accrediting agency
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
English Language Proficiency
Basic Life Support (BLS) certification
U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment
Selective Service Registration is required for males born after 12/31/1959
Subject to background/security investigation
Selected applicants will be required to complete an online onboarding process
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)

Preferred

Experience working with community care providers
Familiarity with in-home care services
Consult management experience
BSN preferred
