Responsibilities
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to implement individualized care plans
Continuously evaluate appropriate, cost-effective levels of care
Coordinate care transitions with physicians, social workers, discharge planners, and claims teams
Arrange non-medical support services (e.g., housing, transportation) to support treatment adherence
Engage specialty resources as needed to improve patient outcomes
Maintain detailed documentation of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Act as a patient advocate while ensuring privacy and confidentiality
Intervene promptly to prevent or minimize adverse patient outcomes
Qualification
Required
Education: Associate or Bachelor's degree in Nursing (or related field)
Attendance: Commitment to mandatory attendance during the first 90 days
Case Management certification
Strong ability to analyze complex medical information and make sound clinical decisions
OASIS and HCHB experience
Multi-state RN license
Current TB test
CPR/BLS certification
Wound care competency
Valid insurance and ability to travel locally
