Responsibilities
Leverage leads and relationships to generate consistent referrals from: hospitals, Skilled Nursing Facilities (SNFs), case management teams etc
Strengthen and expand relationships with: Hospital case managers and discharge planners, SNF leadership and care coordination teams, Physicians, Etc
Represent both home health and hospice services across LA County
Serve as a senior face of the organization in institutional settings
Identify growth opportunities within existing hospital systems and SNF networks
Maintain strong alignment with compliance, CMS, and Medicare regulations
Collaborate closely with leadership to ensure seamless patient transitions
Participate in strategic planning and build out a business development team as the organization continues to scale
Qualification
Required
3+ years of business development or liaison experience in home health and/or hospice
Must have direct experience and relationships in Los Angeles County
Established book of business within hospitals and/or SNFs
Strong understanding of Medicare hospice and home health regulations
Strong understanding of hospital discharge workflows
Strong understanding of case management referral processes
Proven ability to build trust with institutional partners
Entrepreneurial mindset with leadership ambition
Passion for patient-centered, ethical care delivery
Preferred
Known and trusted by physicians, hospitals, case managers and SNF leadership
Values transparency, credibility, and long-term relationships
Wants to grow with a private, reputable organization
