2+ years expNEOGOV is looking for a Care Coordinator to join the NorthPoint Health & Wellness Center, a community-based medical and dental clinic. The Care Coordinator will work collaboratively with healthcare teams to assess, plan, and implement care plans for patients, particularly focusing on women who are pregnant or have chronic conditions.

Responsibilities
Assess, plan, and coordinate patient care according to established clinic policies and procedures, objectives, performance improvement activities and safety, as well as environment and infection control standards
Manage a caseload of patients: follow closely for medical appointments, community resources, and additional needs
Work with the patient and interdisciplinary team to develop and implement an individualized clinical plan of care for the patient
Facilitate collaboration between community health educational resources and the patients to meet the patients’ health care needs
Identify clinic and community resources and coordinate appropriate referrals
Document and monitor patient care visits in EPIC, the electronic medical records system
Maintain accurate records and ensure confidentiality of all data collected during health promotion and prevention services
Qualification
Required
Two years or more of responsible, paid social service experience providing direct client service
A two-year human services certificate
Related education and/or experience to total two years
Preferred
Ability to communicate fluently in Spanish, in addition to English
Experience using electronic medical record systems, such as EPIC to perform data entry where accuracy is essential
Experience providing case management in a clinic setting
Experience working with multiple providers, staff, and collaborative partners to improve the care to diverse and underserved people with a variety of cultural and language barriers
Experience working with Microsoft Office Suite such as Excel, Outlook, Teams, and Word
Knowledge of community resources and social services available to clients
Ability to work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner
