Child & Family Support Services Supervisor (CFSS)

Responsibilities
Provides direct supervision to CFSS staff, including the Senior Case Manager, Medical Case Manager(s), and interns
Oversees daily operations of supportive housing, comprehensive case management, and medical case management services, ensuring continuity and quality of care
Monitors staff caseloads, productivity, documentation quality, and service timeliness
Supports implementation of program work plans, contracts, and performance targets
Partners with the CPO to support program growth, including the expansion of medical case-managed clients
Supports the integration and expansion of medical case management services, including care coordination, referrals, and health-related goal planning
Ensures alignment between medical case management, housing stability services, and psychosocial supports
Monitors compliance with healthcare-related documentation standards, confidentiality (HIPAA), and funder requirements
Coordinates with healthcare partners, hospitals, clinics, and managed care entities to support client outcomes
Reviews case records to ensure accuracy, completeness, and compliance with local, state, and federal requirements
Supports program audits, monitoring visits, and reporting requirements
Implements continuous quality improvement (CQI) practices to strengthen service delivery and client outcomes
Assists with data tracking, outcome monitoring, and internal reporting related to housing, case management, and medical services
Provides coaching, mentorship, and performance feedback to CFSS staff
Supports onboarding and training of new staff and interns
Reinforces trauma-informed, culturally responsive, and equity-centered practices across the team
Identifies training needs related to case management, medical coordination, documentation, and client engagement
Maintains a limited caseload to support program coverage, staff development, or high-need cases
Supports group or individual client engagement activities, workshops, or crisis response as needed
Collaborates with Operations to maintain HIPAA compliance and data integrity
Partners with Finance and Program Leadership to support budget monitoring and resource allocation
Participates in intake, housing placement, and interdisciplinary case review processes
Represents CFSS in internal meetings and select external partner engagements
Qualification
Required
Associates Degree in Social Work, Counseling, Public Health, or a related field required
Minimum of 5–7 years of experience in case management, social services, behavioral health, housing, or medical/social service coordination
Experience working with marginalized and underserved communities required
Demonstrated knowledge of trauma-informed care, housing stability, and medical or health-related case management
Strong organizational, documentation, and communication skills
Preferred
Active clinical licensure (LCSW, LCPC) strongly preferred
At least 1-3 years of supervisory experience preferred
