Youth Aid Panel Case Manager

NEOGOV is looking for a Youth Aid Panel Case Manager to provide holistic, trauma-informed support to high-risk youth and their families. The role involves linking referred youth and their families to community resources and ensuring they complete the Youth Aid Panel program requirements.

Responsibilities

Maintain a work schedule with work hours that include evenings and weekends as outlined below
Attend all Youth Aid Panel meetings with referred youth and family (evening hours)
Weekly follow up with youth and family to ensure completion of Youth Aid Panel requirements (evening hours)
Identify the causal factors for the problem behavior that led to the offense
Link YAP referred youth, their families, and other individuals to community resources to address needs such as housing, health insurance, parent education, behavioral health services, substance abuse treatment, and school related services
Consistent communication with Youth Aid Panel members to provide updates on progress of enrolled youth
Attend monthly panel trainings (evenings via Zoom)
Attend monthly Case Management status meetings with Youth Aid Panel Director
Attend quarterly new volunteer trainings (weekend hours)
Maintain communication with other service providers to keep abreast of youth status and changes to provider schedule
Maintain an accurate file with referral, resolution, final disposition, and case notes for each assigned youth
Identify and participate in new training opportunities that relate to trauma-informed practices and/or youth/family
Operate a computer to send emails, attend virtual meetings, maintain work calendar, and complete agency documentation
Maintain regular contact with team members and share information that will help others work better with YAP youth and families

Required

Bachelor's degree in social work or related field
Experience working with at-risk youth
Good networking and communication skills
Experience providing case management
Ability to work in a changing environment
Valid driver's license and car insurance by date of hire

Preferred

Excellent organizational skills
Good computer and database skills
Experience working with multi-disciplinary teams
