This role coordinates, develops, and implements community-based supported living programs for disabled adults with severe behavioral and medical challenges, overseeing Supported Living Services in the Sacramento area.

Responsibilities
Coordinate, develop and implement community-based supported living program for disabled adults with behavioral and medical challenges
Oversee and coordinate Supported Living Services for the Intellectual and/or Developmentally Disabled (I/DD) adults
Work directly with individuals who have potentially aggressive behaviors in their homes and communities
Handle general administrative duties such as documentation, reports, and supervising field staff
Conduct on-site assessments for incoming clients and provide hands-on support for current clients
Develop and implement plans, goals, and celebrations for the people supported
Supervise field supervisors and staff working one-on-one with clients
Implement crisis intervention plans and write therapeutic treatment plans
Conduct quality assurance checks and fulfill high-supervisory and management tasks
Qualification
Required
At least three (3) years experience working directly with individuals who have Intellectual and/or Developmental Disabilities (I/DD population)
At least three (3) years experience working directly with individuals who have regular and consistent behavioral challenges, including physical aggression, elopement, self-injurious behaviors, and other challenges
At least two (2) years of supervisory and budgetary/finance experience
Understanding of Supported Living Services (SLS) and the Medi-Cal system
Computer and driving skills, including the use of standard electronics, computer and database systems
Being a licensed and insured driver with a clean driving record (no excessive points, and no DUIs/wet-and-reckless in the past 10 years)
Preferred
Educational background (BA/MA) in a related field, such as Psychiatry or Social Work
