Providence Place - Health Informatic Systems Coordinator

Responsibilities
Analyzes all medical records for residents being discharged, reviews and verifies that all documentation is correctly filed for the terminal record
Provides technical guidance to Health Unit Coordinators for resident chart set-up and maintenance
Abstracts and retrieves medical data used for evaluation, planning, survey and investigation
Responds to inquiries from physicians, nurses, staff, outside agencies, and/or individuals regarding medical records (providing appropriate consent for release of medical records has been obtained)
Assist and prepares statistical reports required by external agencies and regulatory authorities
Ensures strict confidentiality of medical records and compliance with “HIPPA” rules and regulations
Schedules MDS cycles for the RAI nurse and transmits the MDS according to Federal and State schedules of completion, reconciles the MDS
Performs a variety of clerical duties involved in maintaining resident medical records to include filing, retrieving, and distributing medical records
Maintains data logs and schedules (MDS) in accordance with established policies and procedures
Sort, file, and collate a variety of resident medical records and information
Examine resident medical records for completeness ensuring all required information is included; note any deficiencies and refer to appropriate area for follow-up
Answer telephone and take requests for medical records or information; sort requests according priority and provide medical record information according to legal consent for lease of information and/or facility policy and procedure
Maintain daily census lists and record resident activity (LOA, admits, discharges, deaths, return from hospital, etc.) check census information against existing records for accuracy and completeness verifying information and making corrections as necessary
Maintains admission and discharge log of residents by dates
Runs off monthly Federal Quality Indicator report and distributes to the DON, ADON, and Clinical Directors
Qualification
Required
High school graduate or equivalent
Three-five years experience
Exceptional telephone and communication skills and the desire to work closely with senior adults is essential
Must be able to relate professionally, positively, and cooperatively with residents, resident's families and employees
Must have proficient knowledge of computer systems, including e-mail use
Must be capable to maintaining regular attendance
Must be compassionate toward the elderly, work with tact and ethical awareness, and ensure personal tenant information is kept confidential
Clear understanding that all injuries/accidents must be reported to a supervisor immediately
Preferred
Experience in a care facility is preferred
