Responsibilities
Provide direct medical care to skilled and long-term nursing home patients under the direction of supervising physician; including but not limited to: obtaining histories, physical exams, ordering and interpreting appropriate lab and x-ray studies, establishing diagnosis, and initiating treatment plans
Proactively manage care to ensure appropriate levels of medical management. Provide patient education to promote wellness and support disease management process within the skilled patient care model
Communicate with the patient’s primary care provider and specialists on plan of care
Complete all other assigned projects and duties as assigned
Qualification
Required
Must be a licensed ARNP or PA
Equivalent to a college degree and a professional certificate or a graduate degree
The ability to motivate or influence others is a material part of the job, requiring a significant level of diplomacy and trust
Obtaining cooperation (internally and/or externally) is an important part of the job
Self-motivated and result-driven practitioner
Preferred
Three years to five years of similar or related experience preferred
Primary care experience preferred
