Social Worker/Case Manager (12)
NEOGOV is seeking a Social Worker/Case Manager to join the Sibley County Public Health and Human Services Department. This role focuses on providing person-centered, recovery-oriented case management services to adults experiencing mental health challenges, coordinating various supportive services, and ensuring compliance with program requirements and regulations.

Responsibilities
Performs, participates in, and conduct intake services for the department, as assigned. Screens and documents incoming calls for a wide variety of social service issues. Assists them in identifying their needs and makes referrals to appropriate external or internal services. Coordinates emergency placements with foster homes, detention and law enforcement, as needed. Prepares maltreatment reports for children or vulnerable adults. Documents and records all interactions in the SSIS program
Collects social assessments and evaluations to determine appropriate types and methods of services, and/or eligibility for services. Performs duties such as: 1. Conducts pre-petition screenings to assess an individual’s risk of danger to self or others. Assesses the need for long-term hospitalization or civil commitment. Conducts investigation, completes PPS documents, and coordinates with County Attorney’s Office, hospitals, court administration, family and client. 2. Conducts screenings to determine eligibility for CADI and DD programs. Determines and assesses eligibility based upon the degree of disability. Interprets social and emotional factors to others involved in treatment. 3. Conducts client and/or collateral contact interviews to perform needs assessments and/or needs for case management services; complete client histories, and to assess chemical use/abuse and makes recommendations concerning level of treatment. 4. Identifies and assesses children who might be eligible for mental health services and assists them in obtaining a Diagnostic Assessment to determine eligibility. Conducts family visits to assess child’s emotional, physical, social, educational, developmental or special needs
Performs case management functions in assigned program areas such as: 1. Provides direct interventions and parenting skills for identified children and families in their homes, schools or in the community. 2. Develops Individual Family Community Support Plans, out of home placement plans, case plans etc. based upon assessments and evaluations. Revises and updates plans in accordance with rules, regulations, department policies and procedures. 3. Reviews cases with internal staff. Collaborates with and assists team members during client crisis. 4. Monitors services, provides care coordination, and monitors the activities of service providers to ensure the proper care and delivery of services. 5. Collaborates with family members, school professionals or other to make sure client needs are met. 6. Makes referrals for services to internal and external agencies. Provides for and/or makes arrangements for appropriate services and referrals (e.g. medical, educational, vocational, etc.) to meet the needs of clients. Provides discharge, consultation and relocation services, as needed. 7. Testifies in court. 8. Supervises out-of-home placements. 9. Implements licensing processes for family child care, child and adult foster care. Orients applicants and conducts background studies; conducts home visits and inspections; approves and/or denies applications. Provides ongoing license inspections and monitoring in accordance with rules and regulations. Investigates any complaints or violations. Prepares corrective action orders and/or recommends the approval or denial of variances
Maintains, documents, updates and enters case management activities in accordance with all program requirements and guidelines. Prepares court reviews and prepare for court hearings. Updates information in SSIS or related department databases, as required. Prepares regular and special reports, as required
Serves on various task forces and/or committees
Serves as a team member on established committees
Attend inter-agency meetings to coordinate social services and monitoring services
Provides training to service providers on mandated reporter requirements, program service availability, drug abuse, vulnerable adult definitions, etc
Performs other duties of a comparable level or type, as required
Keeps abreast of changing trends, policies, best practices and developments in areas of responsibility
Attends conferences, seminars, regional meetings and participates in various professional organizations, as appropriate
Employees must consistently report to work as scheduled and maintain regular attendance
Qualification
Required
BA/BS Degree in Social Work, Sociology, Psychology or close related field
A valid MN Driver's license
No prior work experience is required
After hire, department may expect training and certification in various areas as determined by the department and job assignments (e.g. Rule 79 Training, SSIS Training, etc.)
Skilled in preparing and writing assessment and case management reports and/or client records/files
Skilled in oral and written communications
Skilled in establishing and maintaining effective working relationships with supervisors, service providers, program participants, family members, law enforcement, other governmental agencies and other professionals in the field
Skilled in investigating, interviewing and gathering information and documentation needed in making assessments and evaluations concerning chemical dependency, child and adult mental health, child welfare, adoption services, placements, and/or study reports
Skilled in formulating and developing individual service plans for clients
Skilled in making necessary and appropriate referrals to other agencies, service providers, or community resources
Skilled in testifying in court
Skilled in applying judgment, discretion and decision making in developing strategies and plans to best assist client needs and concerns within relevant and applicable requirements, regulations, rules and policies and procedures
Skilled in conducting ongoing follow-up, case monitoring and reassessments to address changing needs of clients
Skilled in documenting and maintaining required case management activities and requirements
Skilled in working with and collaborating with other care professionals, law enforcement, service providers and team members regarding child protection and case management activities
Skilled in performing general intake services within the agency, as needed
Organizational skills required in determining priorities, meeting deadlines and carrying out case management activities with minimal instruction/direction
Knowledge of the fundamental, principles, techniques and trends in social services and case management
Knowledge of local, federal and state regulations, laws, rules, policies, procedures, and eligibility requirements pertaining to programs of assignment
Knowledge of forensic interviewing, investigative and family assessment interviewing and approaches
Knowledge of mandated reporter training and requirements
Knowledge of individual and group behavior; family systems, case work objectives, principles and methods
Knowledge of record keeping requirements and county administrative policies and procedures pertaining to waiver programs
Knowledge of data privacy, HIPPA and confidentiality
Knowledge of community resources, providers and other resources within the community in areas of program responsibility
Knowledge of general office procedures, computer operations, computer software and applications used within the agency
