Clinical Pharmacist
Decypher is a company dedicated to improving the health and readiness of America’s service members, veterans, and their families. They are seeking a Clinical Pharmacist to support the mission at Naval Medical Center San Diego, focusing on patient care in an ambulatory pharmacy setting.

Responsibilities
Develop and document therapeutic plans, evaluate medication therapy
Order, review, and analyze laboratory test and other diagnostic studies necessary to monitor and support the patient’s medication therapy
Prescribe medications, devices, and supplies to include: initiation, continuation, discontinuation, monitoring, and altering therapy based on established formulary protocols and in accordance with P&T approved scope of practices
Order consults, as appropriate, to maximize drug therapy outcomes and disease state management
Monitor the patient’s response to drug therapy through all available sources, such as, patient a pharmacy records, and laboratory data
Educate and counsel the patient regarding his/her drug therapy and/or protocol
Qualification
Required
Doctor of Pharmacy (Pharm.D.)
Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE) or a Foreign Pharmacy Graduate Examination Committee (FPGEC)
Current, full, active, and unrestricted license to practice as a clinical pharmacist
Basic Life Support (BLS)
Possess a minimum of 2 years full-time experience as a pharmacist within the preceding 3 years of direct patient care in an ambulatory care setting such as the Pharmacy Anticoagulation or Medical Home Port Clinic or Completion of the American Society of Health-System Pharmacists (ASHP) Accredited Pharmacy Residency
Certification by the Board of Pharmacy Specialties in one or more recognized practice areas appropriate for the privileges requested (e.g., Pharmacotherapy, Ambulatory Care Pharmacy, et.); or Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy; or Completion of a Post-Graduate Master of Science degree with emphasis in medication therapy management or direct patient care
