Care Coordinator

Protocol Behavioral Health provides cancer-specific behavioral health treatment to patients by partnering with oncology clinics and cancer centers. As a Care Coordinator, you will facilitate patient referrals and ensure they receive the necessary behavioral health support throughout their treatment journey.

Mental Health Care

Responsibilities

Proactively outreach to patients who have been referred for services (via phone and text) within established timeframes
Provide in the moment support for emotional and behavioral health needs
Clearly explain the care Protocol delivers, answer initial questions, and build rapport
Schedule initial appointments with BHCMs, coordinating patient availability and clinical capacity
Maintain accurate and up-to-date records of outreach attempts, outcomes, and scheduling in our EHR system
Identify common barriers to scheduling (e.g., patient hesitation, logistical issues) and escalate to the team for resolution
Collaborate with administrative staff and BHCMs to ensure smooth hand-offs and timely patient engagement
Track and report on core performance metrics: referral-to-outreach time, outreach-to-scheduled-intake time, and referral-to-scheduled-intake conversion rate
Adhere to all privacy, HIPAA, and company standards for handling patient information

Qualification

Bachelor’s degree in mental healthClinical experience in behavioral healthExperience using EHR systemsExperience using scheduling systemsCommunication skillsEmpathetic mindsetOrganizational skillsSelf-motivatedCollaborative teamwork

Required

Bachelor's degree in a mental health field, such as psychology, counseling, or social work required
Clinical experience in behavioral health
1–3 years of relevant experience, with openness to entry-level candidates who have prior patient-facing experience through work or volunteer roles
Excellent verbal and written communication skills; comfortable initiating conversations with referred patients
Strong organizational skills and ability to manage time-sensitive tasks in a fast-moving environment
Self-motivated, results-oriented, and comfortable working with metrics
Empathetic, patient-centric mindset with strong professionalism
Ability to work collaboratively across teams

Preferred

Experience using scheduling systems, EHR, and CRM platforms preferred
