Responsibilities
Administer skilled nursing care to patients in their residence, as prescribed by the physician
Assist in providing nursing services and identifying patient's physical, psycho-social, and environmental needs
Participate in planning, coordinating, and implementing care in conjunction with the RN
Ensure effective communication with patients/families and document outcomes of interventions
Utilize infection control measures to protect staff and patients
Assist with specialized duties related to end-of-life care for Hospice patients
Participate in admitting, discharging, and transferring of patients
Qualification
Required
Current Nursing License in the Practicing State
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
Current CPR Certification
Willingness to participate in an on-call rotation
Must have a reliable vehicle and active auto insurance (non-negotiable; daily travel required)
