Provide support and stabilization services to youth & families
Coordinate treatment, discharge planning, and case management
Help clients build natural supports and sustainable community connections
Support families with routines, communication, behavior plans & more
Collaborate with courts, schools, DCF & community providers
Attend weekly supervision & team meetings
Qualification
Required
Associate's required in related field
Experience with traumatized youth/adolescents
Valid driver's license & clean background
Preferred
Bachelor's preferred (Human Services/Social Work/Therapeutic fields)
