Adult Mental Health Team Lead

Responsibilities

Work with Program Manager to develop, schedule, and run team meetings
Staff will develop team meeting agendas to include important Center updates as well as addressing any training/education deficiencies or reviewing new policies and procedures
Staff will create and maintain a sign-in sheet for other staff to sign in and acknowledge participation and understanding
If a meeting is missed by a team member, staff will get with them within one week to review the information
Provide courteous, professional services at all times
Handle calls from clients or others related to complaints or problems/concerns with services or treatment and contacts the Program Manager and Rights Department where needed
Maintains professional boundaries
Interact with Center staff, individuals, and community members in a professional manner
Provide Case Management and Rehab services where needed
Case Management and Rehab Services are provided in a team-based environment. Caseload size will vary
Provide skills training as appropriate to individuals identified on caseload approved for services as defined in Texas Resilience Recovery (TRR) guidelines
Provide resources/referrals to meet individual needs and helps them navigate different systems
Will assist other teammates in their absence by providing services to individuals on their caseload as needed
Complete community base visits as required
Community based visits to occur 20% of the time for individuals approved for Level of Care (LOC) 1 and LOC 2, and 80% of the time for individuals approved for LOC 3, dependent upon clinical judgement and needs derived from ANSA
Ensure each individual served is provided with services as required by the level of care assignment for each month
Engagement measure with consumers is 65.6% on a monthly basis
Maintain contact standards as defined in TRR guidelines
Maintain the appropriate productivity level commensurate with overall duties and responsibilities, which may vary
Administer clinical assessments and paperwork to determine appropriateness of level of care assignments required to meet an individual's needs
Required paperwork includes: Adult Needs and Strength Assessment (ANSA), Recovery Plan, Substance Abuse Needs Inventory, MH Consent to Services, TIMA, consent forms, Tobacco Cessation Form, and Columbia-Suicide Severity Rating Scale (CSSRS) Form completed prior to expiration and as needed based on individual change
Is familiar with all applicable Texas Administrative Code (TAC), fidelity tools, and Texas Human Health Services (HHSC) contract information and Center policies and procedures
All documentation is completed within two business days, to meet or exceed 95% monthly
Skills training and Psychosocial Rehab notes will include fidelity words and curriculum used
Ensure individual charts are in compliance with HHSC contract requirements and standards
Completion of 10 chart audits monthly by assigned deadline. May delegate to staff
All paperwork will be signed in black ink with credentials and date
All paperwork is completed within Center policies and procedures
Provides emergency/crisis intervention when necessary, according to HHSC and Center policies and procedures
Ensure each Adult Mental Health Case Manager chart is in compliance with HHSC fidelity tool and provides training as needed
Ensure each Adult Mental Health Case Manager has met their given metrics for client engagement, productivity, and document timeliness report and provides training as needed
Identifies team training needs and develops and provides on the job training individually and in groups as needed
Monitor Adult Mental Health Case Managers staff development training to ensure completed by deadlines given, for on-boarding staff
Provide support and guidance to Adult Mental Health Case Managers
Attend community meetings to discuss services provided
Monitor program Center vehicles related to cleanliness, oil changes and regular maintenance needs, and compliance of use by staff
Participate in the process of interviewing and hiring new team members
Meet with Program Manager of Adult Case Management monthly to discuss team productivity and any issue that would require supervisors' immediate attention
Work with peers to develop monthly/quarterly team training and team building related to topics associated with primary job roles and responsibilities
Foster communication with multiple programs to ensure clients' needs are addressed and ensures staff are kept informed of any updates or changes
Engage in real-time chats to provide staff support and proactively address any issues that may arise
Generate weekly reports to monitor staff productivity and identify potential billing issues
Complete other duties as assigned
All assignments given are completed in a timely manner
Has the ability to work independently and make decisions but seeks guidance when needed
Participates in the Center's Zero Suicide Initiative
Supports individuals experiencing a crisis by promptly connecting them with appropriate Center programs, such as the Mobile Crisis Outreach Team (MCOT), Psychiatric Triage, Crisis Prevention services, and/or other applicable resources. Ensure seamless coordination to facilitate timely intervention
Remains with individuals, when deemed safe, to provide support and maintain a calm environment until specialized crisis staff or emergency personnel arrive to take over intervention efforts. Adheres to safety protocols to prevent escalation of the crisis situation
Participates in debriefing sessions with the immediate supervisor or designated staff following a crisis event. Collaborate with the team to review the incident, identify lessons learned, and ensure all HHSC guidelines and Center policies are followed

Required

Graduation from an accredited four-year college or university with major course work in human services field as required to qualify as a Qualified Mental Health Professional (QMHP), required
Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC)
Must possess and maintain a valid driver's license and automobile insurance
Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days
Successful completion of all position-specific training within thirty (30) days of employment is required
Knowledge of community resources, of case management delivery systems, and of program policies and procedures
Basic knowledge of Microsoft Word, Excel, and PowerPoint
Skill in active listening, de-escalation techniques, empathy, and basic knowledge of social services systems
Skill documenting services provided, conducting assessments, and developing recovery plans
Skill in communicating with a variety of individuals from various backgrounds and experiences
Ability to assess clients' needs, coordinate individuals' services, communicate effectively, and to train others
Ability to work as a member of an interdisciplinary team to ensure referral to appropriate Center program as needed
Ability to work independently and make decisions but seeks guidance when needed
Ability to make decisions based on education and or clinical experience, which reflect consistency with Center's Guiding Principles, Policies and Procedures, and Standards of Care
Ability to work with clients of different cultural backgrounds, sexual orientation, and belief systems
Ability to work with clients who may display symptoms of mental illness including; poor hygiene, difficulty controlling anger and low frustration tolerance, difficulty or inability to display appropriate social behavior, and who are suicidal or homicidal
The ability to engage with individuals served in community locations, including but not limited to residences, community businesses, and local hospitals
Understanding and practice of Trauma Informed Care principles

Master's Degree in a human services field, preferred
Previous Crisis experience preferred
English/Spanish bilingual preferred
Experience: Six (6) months prior experience in a mental health field preferred
