Responsibilities

Provide direct supervision and clinical oversight for a localized, multi-disciplinary team supporting members living with mental health, substance use, and intellectual/developmental disabilities
Establish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member’s physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently
Monitor teams to help ensure integration of care through the establishment of a multi-disciplinary team (e.g. member, caretaker/legal guardian, PCP, behavioral health provider, specialists, nutritionists, pharmacy, etc.) to meet the member’s needs
Be familiar with evidenced-based and best practices for members and translate that knowledge into specific processes, procedures, and workflows for staff to identify and facilitate effective and efficient coordination of care for members
Use knowledge and skills to create and identify methodologies and tools to be used in the execution of care coordination
Coach team members to use member engagement practices that support the engagement of members throughout the continuum of care

Qualification

Master’s degree in Human ServicesLicensed in North CarolinaBehavioral health experienceSupervision experienceCoaching skillsTeam leadership

Required

Master's degree in a Human Services field and three (3) years' experience working with behavioral health, mental health, substance use, and/or intellectual/developmental disability populations. **OR**
Equivalent combination of education/experience
Fully licensed in the State of North Carolina as an LCSW, LCMHC, LMFT, or LPA
Must have a valid driver's license
Must reside within North Carolina
Must be able to travel within catchment as required

Preferred

Work experience focused on working with individuals with behavioral health, mental health, substance use, and/or intellectual/developmental disability populations
Supervision of two (2) or more employees
