Home Health Clinical Coordinator (LPN)

Responsibilities

Reviews clinical documentation to help determine home health appropriateness
Assists in compiling and reporting on clinical metrics shared with community partners
Tracks and updates team members on new hospitalizations, hospital holds and resumptions of care
Must be self-driven with a high degree of attention to detail
Communicates with patients, families and caregivers regarding home health services
Coordinates with physicians and referral sources (as appropriate) regarding home health plans of care
Assists in ensuring that patients are seen in a timely manner and works with physicians to receive updated start of care orders when necessary
Cross trains to provide back-up coverage for scheduling and intake coordination when needed
Assist with field visits if needed
Uses equipment and supplies effectively and efficiently
Participates in personal and professional growth and development
Other tasks and responsibilities as assigned

Qualification

Licensed Practical NurseBLS CPR CertificationHome Care ExperienceCommunication SkillsOrganizational Skills

Required

Graduate of an accredited practical nurse or vocational nursing program
Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of service
Complies with accepted professional standards and practice
Demonstrates good verbal and written communication, and organization skills
Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order
Possess and maintains current BLS CPR Certification

Preferred

One (1) year nursing experience in home care, hospice care, institutional hospital or facility care preferred
