Responsibilities
Under the supervision of the Outreach Case Management Services Coordinator of Adult Mental Health Services, serves as a professional performing a wide range of direct and indirect mental health services to consumers with serious and persistent mental illness or co-occurring disorders
This position serves as a part of a multidisciplinary team providing assessments; treatment planning; case management services; crisis intervention; mental health services; referral and linkage to various other CMHA-CEI programs and/or services in the community; outreach services to consumers with multiple needs; and facilitation of group activities
Responsible for detailed case documentation
Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures
Qualification
Required
Bachelor's degree in Social Work, Psychology or other related field required
A minimum of one year of professional experience with adults with primary diagnoses of a serious and persistent mental illness and/or co-occurring disorder is required
On a case-by-case basis, may assess and consider an applicant with two years of paraprofessional work experience with adults with primary diagnoses of a serious and persistent mental illness and/or co-occurring disorder
Must be able to operate a computer and have the ability to gain proficiency in CMHA-CEI's computer systems
Ability to communicate effectively both in writing and verbally is required
Possession of a valid Michigan driver's license and access to reliable transportation for job related use is required
