Responsible for analysis of SOC, Recertification and Resumption (and Other Follow-up OASIS) OASIS and accompanying documentation, management of proper utilization of resources to ensure positive patient outcomes and optimal financial outcomes
Covering team member PTO, and other requested office tasks
Qualification
Required
Oasis and Home Health Coding Certifications
Case management experience
Associate degree in nursing
Bachelors degree in physical therapy
OASIS Certification
Home Health Coding Certification; must obtain within 12 months of hire
Registered Nurse or Physical Therapist License in the State of Florida
Valid Florida drivers license within 30 days of employment
Communication and interpersonal skills
Basic computer skills
Customer service skills
Preferred
Home Health Case Management experience
Bachelors degree
Masters degree
Two (2) years of Case Management
Two (2) years of Home Health case manager experience
