Responsibilities

Assess patient eligibility and care needs for home health services

Develop, implement, and revise individualised, patient-centered plans of care

Provide skilled nursing care per physician orders and agency protocols

Perform medication reconciliation and communicate issues to the care team

Coordinate care with physicians, therapists, aides, and interdisciplinary teams

Supervise Home Health Aides and complete required supervisory visits

Educate patients and caregivers on disease management, safety, and emergency care

Document all care accurately in the EMR and meet productivity standards

Ensure compliance with infection control, QA, QAPI, and PDGM requirements

Qualification

Active RN LicenseHome health experienceClinical skillsComputer skillsCommunication skills

Required

Active Oklahoma or Multistate RN License

Minimum 1 year of home health or related nursing experience (required)

Valid driver's license, reliable vehicle, and acceptable driving record

Strong clinical, communication, and computer skills

Benefits

$10,000 Sign-On Bonus (2-year commitment)

Mileage reimbursement at $0.70/mile
