Admission Coordinator

The Admission Coordinator will guide patients and their families through the admissions process, ensuring compliance with hospice care regulations and delivering superior customer service.
Responsibilities
Guiding patients and their families through the admissions process
Ensuring compliance with hospice care regulations
Delivering superior customer service
Effective communication with healthcare providers
Gathering medical information for eligibility assessments
Facilitating the admission process
Coordinating services
Ensuring accurate documentation
Facilitating training as needed to maintain compliance with Medicare regulations and standards
Qualification
Required
Strong Communication and Customer Service skills, with the ability to empathize and build relationships with patients and families
Familiarity with Medicare guidelines and a solid understanding of compliance requirements
Experience in Medical Terminology and the ability to accurately process and assess relevant patient information
Ability to facilitate Training and maintain knowledge of current hospice care standards and practices
Detail-oriented and proficient in documentation and administrative processes
Preferred
Associate's or Bachelor's degree in a related field is preferred
Previous healthcare or hospice care experience is highly desirable
