Responsibilities
Providing preventive, acute, and chronic care for older adults
Conducting in‑depth assessments and managing individualized care plans
Ordering and interpreting labs and diagnostics
Collaborating closely with Nurse Practitioners, Physician Assistants, behavioral health, and palliative partners
Participating in population health and quality improvement initiatives
Supporting and mentoring advanced practice providers within your pod
Engaging in ongoing clinical collaboration, coaching, and professional development
Qualification
Required
MD or DO with an active California medical license
Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
Comfortable practicing in a team‑based, home‑centric care model
Values autonomy, accountability, and long‑term partnership
Compassionate, communicative, and committed to continuity of care
Preferred
Experience caring for seniors or medically complex patients
