DOHMH Housing Program Manager

Bridging Access to Care, Inc. is a nonprofit organization dedicated to building healthy communities through behavioral healthcare services. The Program Manager for DOHMH Housing will lead the NYC 15/15 Scatter-site Housing Programs, supervising case managers and ensuring the delivery of comprehensive, person-centered services to tenants.

Responsibilities

Oversee all DOHMH NYC 15/15 Scatter-Site housing case management services with a strong focus on safety, permanency, and well-being
Monitor program deliverables and ensure full compliance with city and state requirements
Conduct weekly client contact reviews and comprehensive monthly chart audits for each case manager
Assist in developing and updating program policies, procedures, and service frameworks
Complete required reports for DOHMH (TAD, LOS, TMS)
Supervise case managers and interns, providing weekly individual supervision
Participate in staff hiring, evaluation, scheduling, and corrective action processes
Train and orient new staff on responsibilities, mandates, assessments, and documentation standards
Ensure all staff meet training requirements and connect them to additional learning opportunities
Participate in internal and external case conferences, team meetings, and department meetings
Offer crisis intervention, de-escalation, and conflict resolution services
Step in to provide direct services when staffing shortages occur
Use trauma-sensitive tools (safety plans, psychoeducational groups, red flag reviews, etc.) to support a nonviolent, trauma-informed environment
Participate in the 24-hour rotating on-call schedule to provide after-hours crisis support
Ensure all client assessments and service plans are thorough, client-centered, and completed on time
Emphasize harm reduction and motivational interviewing practices throughout casework and team collaboration
Plan and facilitate group programming including tenant meetings, workshops, community-building activities, and recreational events
Attend provider meetings and related community events as assigned
Support case managers with high-level interventions when complex issues or crises arise
Maintain accurate documentation including progress notes and required program records
Manage timesheet completion and time-off requests in the Paycom system
Perform other duties as assigned to meet evolving program needs

Required

Bachelor's degree in social work, psychology, behavioral health or human services degree required
2+ years of supervisory experience required in lieu of Master's Degree
Minimum 2+ years of direct service with the population of focus required
Bilingual applicants strongly encouraged to apply
Crisis Intervention: 2 years (Required)
Plan and facilitate group programming: 2 years (Required)
Monitor program deliverables: 2 years (Required)
Ability to Commute: Brooklyn, NY 11226 (Required)
Willingness to travel: 50% (Required)

Preferred

Master's degree in Social Work, Mental Health Counseling, or another related field preferred
