Clinical Pharmacy Specialist - HBPC

Responsibilities

Performs comprehensive medication assessment of medication therapy initially, quarterly, and when clinically indicated, to include recommendations that ensure a custom approach that supports patient preference and adherence
Provides comprehensive medication and disease state management services and prescribing medications and treatments in accordance with their individualized scope of practice, as described in VHA Handbook 1108.11, Clinical Pharmacy Services, or subsequent policy release
Reinforces and provides education to the Veteran and caregivers on the proper use of prescribed medications, in collaboration with other HBPC team members. Education may include but not limited to proper storage, administration, dosing and indication for use, side effects, disposal, and expiration dates
Provides a consultative role to facility leadership when a medication error occurs as a result of a VA system's error
Identifies patient-specific medication issues to include, but not limited to, drug interactions, adverse effects, efficacy, appropriateness (indication for medications), and adherence problems
Participates in home and telehealth visits, when clinically indicated and agreed upon, for comprehensive medication or disease state management or medication safety concerns
Provides continuous pharmacy/medication education and reference materials for the HBPC team
Assists in the training of clinical clerkship students during four to eight week rotations
Responsible for coordinating and supervising the activities of postgraduate residents in pharmacy practice in the capacity of preceptor for HBPC rotation throughout the residency year
Provides therapeutic lectures to pharmacy students and residents in cooperation with Associate Chief, Clinical Pharmacy Services, first-line supervisor, and/or special project coordinator
Serves as an authoritative information source on medication and their uses. Provides both written and oral drug information to physicians, nurses and other associated healthcare professionals as well as to patients and their caregivers

Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree or on track to complete advanced degree from an accredited program by the time of hire
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters

Preferred

Completion of PGY1 Pharmacy Residency or equivalent
Completion of PGY2 Pharmacy Residency in Ambulatory Care or Geriatrics
Board Certification (BCPS, BCACP, BCGP)
Experience working under Comprehensive Medication Management Scope of Practice in HBPC or Ambulatory Care
Experience working in the HBPC setting
Leg Six Sigma Trained or Certified (Yellow, Green or Black Belt)
