This role involves building referral partnerships, increasing awareness of hospice services, and supporting census growth through community engagement and education.

Responsibilities
Build and maintain positive, trust-based relationships with referral partners such as hospitals, case managers, social workers, skilled nursing facilities, physician offices, and care managers
Manage and track referral relationships, completing at least 8 meaningful touchpoints per day
Coordinate consents when appropriate, working closely with intake and clinical teams
Plan, organize, and host community and company-sponsored events (e.g., Toy Drives, Memorial Services, Fallpalooza, volunteer initiatives)
Support and coordinate volunteer engagement activities within the community
Plan and present hospice education programs for referral partners and community members
Attend and actively participate in professional organizations and meetings, including IL COC and CMSA Care Manager meetings
Conduct in-person visits with case managers, social workers, and interdisciplinary partners
Create and distribute email marketing communications on behalf of the organization
Represent the organization at networking events, health fairs, and community outreach opportunities
Collaborate with leadership to review referral trends and support growth initiatives
Ensure all marketing and outreach activities follow Medicare, state, and federal regulations
Qualification
Required
Bachelor's degree in Marketing, Healthcare Administration, Communications, or a related field
Experience in hospice, home health, healthcare marketing, or community outreach
Friendly, engaging, and confident communicator with strong relationship-building skills
Organized, self-motivated, and able to manage multiple priorities with flexibility
Comfortable with public speaking, event planning, and professional networking
Knowledge of hospice services, referral processes, and compliance standards
Proficient with email marketing platforms, social media, and CRM or tracking tools
