Deliver clinical care in Nursing Homes, Assisted Living, and Memory Care facilities
Collaborate with a multidisciplinary team to ensure quality outcomes

Maintain accurate documentation and communicate effectively with care teams
Qualification
Required
Current, unrestricted Oregon PA license (or ability to obtain)
Preferred
Experience in geriatrics or long-term care preferred
