The role involves working closely with clinical managers to ensure clients' needs are met according to their care plans while delivering top-quality nursing care at home.

Responsibilities
Performing assessments, monitoring clients’ conditions, and documenting observations, interventions, and evaluations
Providing the initial 4-6 weeks of skilled nursing care for new admission clients
Traveling to clients' homes and staying there for the full shift
Executing and properly documenting doctors' orders
Qualification
Required
Active New Jersey LPN license in good standing
CPR Certified
Willingness to work across market service area (Bergen, Passaic, & Hudson County)
Willingness to work with all ages (Infant through Geriatric)
Willingness to work in all private duty settings (In-Home & In-School)
One year of skilled nursing experience
Hands-on tracheostomy and ventilator experience
Pediatric experience
Commitment to a minimum of two twelve-hour shifts per week
Commitment to work one weekend per month
